We are getting in touch ahead of your upcoming nasal flu vaccine clinic for students aged 2 to 17. Thank you in advance for your continued support in helping us run this safely and smoothly. Consent Form – Action Required Consent Form: https://edupharma.paperform.co/ Consent closes 48 hours before your clinic. Once closed, we will send you a full list of students with completed consent, broken down by class or year group. No student will be vaccinated without a completed consent form. Documents for Parents
- Flu Vaccine for Children Aged 2–17 – HSE Leaflet
- HSE Funded School Vaccination Programme – Information Letter
- Patient Information Leaflet – Fluenz Nasal Vaccine
These documents explain the programme, how the vaccine works, and answer common parent questions. If you or a staff member experiences difficulty accessing the consent link using a school network or shared device, we recommend trying again using mobile data or a personal connection, as school firewalls may sometimes block external forms. This should not affect parents at home, but if anyone continues to experience access issues, please let us know and we will assist directly. Thank you again for your support with this important public health programme. Please don’t hesitate to get in touch if you need anything further.
